If you use the program I referred you to earlier(IconEdit32), it does it very easily. On the right side of the window, you can quickly switch between all of the different formats and they get saved to a single .ico file. I believe it goes from 48x48 @ 16-bit down to 16x16 two color and you can definite an image for each "mode".
