The discussion on the process of getting things fixed has been very interesting, but very encouraging.

FWIW, I have found a workaround. After appending a record and inserting
its primary key, perform a save to commit the addition to the table. Then read the appended record using its primary key. If it isn't found, abort the operation, otherwise proceed to insert the rest of the data and continue as normal. I had some concerns that the appended record might still be hanging around in the cache, but this was not so. Tested it by corrupting the header and the failure is detectable immediately.
