I think the error is around this:

Message:< BR >< TEXTAREA NAME=Notes ROWS=25 COLS=120 WRAP=Virtual >< /TEXTAREA >

Make COLS a bit smaller.

It's probably related to the fact that I'm using LARGE fonts but I shouldn't have a problem because of that, should I?
