It might be, that none of available utilites can fix this problem and you need to resort to latest backup. Try Yuri's Rubinov utility from Download section here.

Also I believe, Cetin Basoz published some way to fix form in one of the thread recently, but I don't recall thread details...

Can you open your form as a table?
