I have had a lot of success with Visio2000-Enterprise Edition. You can use the database mapping wizard, give it a DSN and you're set.

On a recent project, I actually used it to go the other way (to create a MSSQL7 database). I drew the tables, created indexes, and printed out diagrams (with FK crow's feet and direction) that I went over with the rest of the dev team and when I was done it created everything that was diagrammed. Nice. I even got it to create a VFP6 setup once, have to try it with 7 to see if it's better (it was kind of flaky with 6).
