If regions are rectangular check FoxyClasses sample for coolbutton. If not think of keeping vertexes instead and check where clicked (if you have an activex like MapObjects this would be easy directly querying 'geographical' data/zooming, panning, adding, removing, arranging layers - if not I'm afraid it's expensive).
PS: Images in general fields doesn't sound to be feasible to me. If possible keep as external files.
