No, I'm sure it's not It's been around a long time, and occurs when a table header becomes corrupted. Doesn't happen often, but when it does, data is lost without the problem being reported, and continues to be lost until the table is repaired.
Obviously, it can happen through the usual hardware/network failures, though I suspect it is more often related to timimg issues and internal lock settings. The cause of the problem though is less inportant than the problem not being reported when it occurs so that remedial action can be taken promptly.
