Most often this is seen when the netbios name and workgoup parameters in your smb.conf have the same parameters. Use different names for the server netbios name and workgroup name (possibly even need unique user names). "The Microsoft World (tm) has a flat namespace, and an even flatter one in NetBIOS Land. In NetBIOS, users, servers, workgroups and more all exist in the same namespace.
Whoever registers the NetBIOS name first, be it the computer or the workgroup, the other will be unable to do so."

Also consider: encrypt passwords = yes (a wild additional guess)
