Joel,

You'll get error number 18458 (All licensed connections are in use) from SQL Server, whenever someone tries to connect when the number of licenses are maxed out. I know that this doesn't specifically answer your question, but when you periodically get this message it basically means that you need to purchase new CAL licenses.
