It is very difficult to say what causes this. Typically it does not happen, in some cases it may be the fault of operating system. Files sometimes get cross-linked, which can damage your form. It might be even bad hard drive or hard drive controller or some unstable drivers which cause hang-ups. You may never be able to figure out the reason.
As to recovering, once my form was damaged like this, and I had a lot of code there. I was able to open my .FPT as a text file with just Notepad and save my methods code by copy/paste. Then it was much easier to recreate the form.
