I delete anything without opening it for which I'm not familiar with the sender. I'm sure I lose a few "good" messages, but it's worth the trade-off. They are getting even trickier lately as I've received mail where the sender was displayed as my own address! Needless to say, even though I was familiar with the sender, I still deleted it.
