I found the problem. The SecurityObj.cParentBizObj on cUserFormAccessCntrls was set to 'User'. The actual name of the bizobj is 'User1'. I'm not sure how it got that way, we may have removed it and readded to get the 1 on the end, but you may want to check the framework.
