It would be better if I knew the version of the toolbar that you were using, however, let me take a stab at it. In the 3.0 version of the toolbar, you had to make the toolbar part of a formset. This formset had to be instantiated before you open another form in the formset.

I've found in the newer version that I no longer need to do that. I can include the toolbar right on the form, however, because of a bug, you need to add the imagelist for the toolbar by code in the forms init method.

Doing it this way I have been successful and have not received the error message that you are indicating.
