Hi,

In our unit we use TELEform to read surveys. TELEform creates file in any format you want (dbf, mdb, etc.) and loads all the data in it. Our application just processes this file (mdb in our case) and uploads it in our system with all data validation.
TELEform is quite expensive and license based, but on long run it does good job. I should mention, that it relatively slow (takes 1 sec per page to OCR.)
