Does anyone know a good reference (book, articles) that discusses
- Change control methodology
- Distribution, installation, implementation, and upgrade guidelines
- Development environments (such as development, test, and production servers)
for 3-tier C/S and n-Tier web applications?

We recently severed a contract with a third party vendor and will now develop our application in-house. I was assigned the task to develop detailed Infrastructure Requirements and I am sure there is someone out there that has written about the subject.
