You're very fortunate. I know what to open and what not to open and I've been caught a few times. Fortunately, I do run a virus scanner, so every time but one in the last few years, viruses that arrived on my system were caught without infecting it.
Maybe my situation is different than yours in that I exchange a lot of documents (mostly Word, occasionally Excel) with other people. I have received a number of infected documents (including several times from people at Microsoft) since macro viruses came around.
I wouldn't work without a virus scanner.
