You may be able to pull it off without literally putting the different field types on top of each other...you may be able to stack them vertically and use "remove line if blank" to get the desired effect. The third table will be added to the select statement. There are two common pieces of information in all three tables, namely, Date,and PhysRef (a unique number that is generated each time a new physical is done).
