You can't. Header and footer settings are user options defined in the client PC's registry. Also when the user is printing he can choose to remove header and footer from the Page Setup dialog.

The only way is to have total control of the user's PC. See MSKB article Q236777 for info on that.

No se puede, el encabezado y pie de pagina son atributos que controla el usuario ya que se guardan en el archivo de registro de Windows. El usuario puede cambiar estos valores al momento de imprimir con la opcion de configuracion de impresion (page setup en ingles).
