I used all of them. However, I have to say, that in 1998 with an NT box, you will be better to use IIS. It comes with NT and you will be able to handle nice features you will have difficulties if you go with another server.

You can use the same approach as you use in desktop development. Working on a framework will allow you better flexibility for handling multiple applications. I will also suggest you go with XWorks Enterprise as you will be able to assign server name to your Visual FoxPro server and handle custom servers for each application. Try to also implement a dispatch server.
