I received the same e-mail and participated in the survey..
BTW, I found few problems with it:
1) Couple of questions do not have all possible choices for the answer...

Say, in first questions I answered, that I don't use VFP7 yet, and lately I was asked about something specific to VFP7.

2) Also this survey could not be closed properly (it gives a bug, which I didn't try to debug).
