It's the Microsoft Common Dialog Dialog Control 6.0. Under Tools/Options/Controls you'll find it listed. Simply check the check box. When you activate the ActiveX controls toolbar you'll be able to drop it on a form. You should then be able to access the help file via the context menu that will appear when you right click the control.
