Use a form to gather user input. Store the input in the properties of any object [either the form or a gloabl app object] or private variables. Then in the report, you can put fields on the report that have those properties or variables as their data source. Your report can even use the values of textboxes and other controls on the form as sources of data values for your report. Although I would rather some data values of a report be derived from some custom report object instead of a form. That way all your reports can be designed to depend on a single report object instead of a variety of forms.
