I've used a product called ClipVoice along with Dialogic telephone boards and was very happy with the result. But this was 4 years ago. If you can find this product, I think it will work for you. You could run a DOS/Clipper "polling" program that would read the numbers and make the call. This program also captures keypresses ("press one for tech support, two for account balance, etc) if you have a need for that.
VBVoice also looks promising, plus it runs in Windows (of course), but I haven't used it beyond the simple demo that comes with it.
