pdf documents are not really designed to be edied, the pdf fomat is more of a complted documewnt which unless you have some way of 'opening' it and recreating it (like acrobat).
So if you look at the 'form' your customer gave you, and see if they have the orig form you can work from... Word Doc whatever) then create the pdf on the fly merging in both the form , and your data.

Another thought would be to conver the pdf into a postscript file. .ps file, and then do a textmerge replacing fields with values in the postscript file.

you would need to get acrobat and define where the fields were... then save the doc as a ps file, run textmerge on it and then convert the ps to pdf
