It is possible that a critical section of the hard disk is physically damaged, and can't be recovered.

I do hope you have a recent backup. If you don't, see what you can recover, booting from another machine (connecting your damaged disk as a secondary disk), and see if you can format it.

Perhaps you should leave this as a last ressort - possibly other members of the UT can come up with a safer solution.
