Addendum: Check the clocks on the machines involved, make sure they are all in good sync. I just remembered that was a symptom of the server dieing, the clock started going haywire just beforehand when the files didn't close right. Now (a few years later) that I know more about how things work, I know the server/workstation clocks MUST be in close sync to get the files to close properly with WRITEAHEAD and whatnot, with your OS and protocol. That's a simpler possible cause, check it also.
