Can anyone tell me why some forms remain behind active(& modal) forms even after these forms have been released?

This phenomenon occurred to me a couple of times. Previously, I thought I had forgotten to release them but, when I checked again, I found the RELEASE code to be there. I can't seem to pinpoint the source of this occurrences.
