OK, for patient details IME there are 2 ways:

1) batch- every xxyy mins/hours the other system produces a message to you with all changed records.
2) Real-time Push- every time patient details are entered/changed or a patient is admitted/discharged, a message gets generated.
3) Real-time Pull- every time your app views/uses patient details, you request an update if the details are more than xxyymins/hours stale.

IMHO it is hard to do without 3) - patients drop in unexpectedly and attend clinic, you want to be sure without fail that your details are current and something does not sneak through.
