For example, the Royal Australasian College of Surgeons (no fools) run very well attended training sessions in SEA rather than demanding people come to Australia/New Zealand- which is a lot closer and cheaper than the US, by the way. They also allow you to sit your exams in larger SEA cities. It works very well.
