It looks like custom printer forms're stored in the registry under
HKEY_LOCAL_MACHINE\SYSTEM\ControlSet001\Control\Print\Forms
HKEY_LOCAL_MACHINE\SYSTEM\ControlSet002\Control\Print\Forms
HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\Print\Forms

I guess, you can export those entries on your PC and import on the workstations.
