Can someone PLEASE give me a short step-by-step tutorial on how to set up a single column list box inside a form class that lists the value in an array that is derived from a query and then pass the selected value to a method that will call a procedure using the selected value as a parameter?

The query is:
SELECT ma_name FROM LMMAILER INTO ARRAY _lmmailer. ma_name is C(40).

I need this for work and not being able to do this simple thing is totally ruining my holiday.
