This is probably due to the next problem -- SDT doesn't think the fields are linked to the Field Repository.

I'm not seeing that -- when I drag a field from the Field Repository, the "Linked to field repository" is enabled (it's normally disabled) and checked. If it's unchecked, that means you don't want it to be linked.

First, what version of SDT do you have -- the latest is SDT 6.0c.
