FYI, Visio is a great product to use. Once you create the diagram, there is a wizard that will define and create the database, tables, relations, etc. from the model.

I came across an article in SQL Server Magazine that seemed to indicate that you could create a database diagram from within the Server Explorer window. Is that only with the Enterprise version, because I could not figure out how to do it.
