Yes, as an example app it is not supported by Microsoft product support. There isn't any kind of explicit or implicit obligation that Microsoft has assumed for developing or improving the FoxISAPI.dll

Yes... and no.
Why no: As a demo application it is not recommended to use it, if you do not know exactly what it does and what are its limitations.

Why yes: ISAPI is standard API for creating MS web server (IIS) extensions. I believe that West-Wind Web Connection was started as version of FoxISAPI and after a lot of code improvements and polishes, now it may (or may not) have some parts of the FoxISAPI code. What I am sure is that WC.dll utilizes ISAPI as the FoxISAPI.dll. The advantage of wc.dll compared to FoxISAPI.dll is that it is a supported extension. Moreover wc.dll has some cool features like using not only COM messaging like FoxISAPI, but file-based messaging and ASP Scripting conext messaging also. The advantage of FoxISAPI.dll is that you have full source code and you are free to make improvements.

Yes, it is - see http://www.compuneed.co.za/ it is implemented using FoxISAPI (or modified version of it). Many people do not encourage themselves to use FoxISAPI because it is harder to maintain it. I do not think that MS will begin to support FoxISAPI - this because if they do it, they might blow out products like West-Wind Web Connection and FoxWeb... (Well this is not certain to happen, but even possibility for competition offence is considered by MS)
Also MS focuses its development forces over ASP.NET, so that if you wish to use supported by MS ISAPI extension - use ASP.NET
