In my experience, yes. Depending on the products and their destination, there may be other taxes involved that are calculated differently. I just haven't had to deal with any yet.

Both. Here in NY, the state has 4% on most purchases. Counties add 2-4%, and local municipalites may also add on a piece. Some items are exempt from state but not local, fully exempt from both, etc.

Depends on the item purchased and its usage. Categorizing inventory in such a way as to allow matching of the customer to products is how I handle this. If a customer buys "A" type of product, he pays X.XX% tax, "B" type of product, Y.YY%, "C" may be exempt or a combination of several rates and so on.
No simple solution to this problem.
