I exported from Outlook 2000 to Excel 2000 and imported back to see if I could find and gotchas. It worked for me, but you want to look at the field mapping and make sure you're pointing those right. I used the default.
I did notice in Excel that there were 2 columns that contained the email address. They're labeled EmailAddress and EmailDisplayName. If you don't have both of those cells in Excel filled in it may be that the addresses are there and mail goes to them, but you don't show it.
I have no idea why anyone would want to do that, but based on the field being duplicated, it looks like it's an option you may be doing accidently.
