It turned out my problem was that delete.bmp is also a framework bmp and was included in the EXE (which is where it should have been) - our specific error was "Unknown COM Error" - I simply re-named the delete.bmp on disk that I wanted to APPEND GENERAL with and all is well now. Not sure if this relates the the problem you were having or not.
