You turn off caching via the System Control Panel Applet - the Performance Tab, File System button, under Troubleshooting; disabling Windows caching is seen as an extreme measure to ensure compatibility with the default file system when delayed writes fail to be enforced properly by the hardware. It will definitely affect the overall system performance, most noticably access to the swap file, and apps that perform frequent random updates of the same small regions of specific files, since disabling the cache forces a physical write to complete each time an update occurs.

If data is held in cache and you reboot the system, the cache is never given the opportunity to update properly, and as a result, data that is being held pending a free period for updating the disk is lost. Disabling delayed writes avoids the buildup of data pending update, but at the cost of forcing the OS to complete writing to disk before continuing after a write.
