Your best bet at this point is to save a copy of the SCX and SCT file, and then use one of the many DBF repair tools such as FOXFIX (you can find several in the downloads section, as well as commercial offerings) to try to fix things up; often, the error is a single byte in the header of the file, which can be fixed by these sorts of tools.

If there's no backup and you can't repair the damage to the Form file, which is essentially a specialized form of DBF, you'll have to start from scratch.
