What is the OS? What are the desktop settings? Does it work on other workstations? Sounds like silly questions, I know, but I had this happen once because the user had changed the desktop display settings to the jungle theme (go figure?). If you set the display settings on the desktop back to windows standard (appearance tab), do the other fields print then? What if you change the font?
