We say that Healthcare is "free" to the user in a socialised system because that eliminates one of the major barriers to quality healthcare for all citizens. Such care is paid for by tax, but take it from me that my taxed healthcare "cost" in a socialised medicine is a fraction of what it is when I am in the US. That goes for me and my staff.
IMHO the very best care comes if there are strong public and private healthcare systems. Society decides what it will pay for, those who want extra or demand better service, need only step across to the private system and pay for it.

"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us."
