John, all of the .EXE COM servers work with DCOMCNFG to allow users other than local to use their services. Not needed for .DLL vfp servers.
Once you've set up the .EXE COM pdf server with DCOMCNFG, unload the avfpdemo2 app or do an IISRESET and try again.
