Steve, - a wild guess here but if you are trying to generate a report that is the equivalent of that form when it is filled out you can scan the form and use it as a "background" for a FoxPro report and then just locate your report controls over the places on the report where they belong. Otherwise, I think Hilmar's got the right idea - start with a template, maybe add bookmarks where you will insert text, automate to open the template, fill in the text and then Save as a new file name.
