This has changed recently (I think starting in the October release of MSDN).
The MSDN viewer has been gradually getting less and less usable over time. THe UI people in Microsoft's Help division are a bunch of bozos. The entire HTML Help system is based on a great concept, but has always been plagued by horrible execution and implementation especially from the help developers point of view.
MS knows of most of these problems but they are not going to fix them. And now HTML Help 2.0 has been delayed until the next release of Windows (Longhorn), whcih means we'll be stuck wiht this screwy help engine for a while.
