Try modifying the report and add a field object that has the exact same expression as what you are putting in the Picture From File area. This way you can see exactly what is returned by that expression and maybe somehow it's not what you are expecting.

I'd also follow the advice of a few others and make sure that you don't have a file in the project with the same name .. or at least exclude it.
