The EXE cannot see the individual forms in tbe APP. I think the command goes something like this:

DO FORM XXX IN ABC.APP

The above command works for PRGs but I am not sure it works for FORMS. I do not have VFP on this Machine otherwise I would have confirmed it.

How ever, I you issue the DO FORM XXX command from inside the APP, it will work.
