I too suspected a hardware problem, but with nothing in the event logs, and no device errors or misconfigs, ya gotta look elsewhere.

I may be an old fool, but I've always stuck with the "RAM + 12 MB" axiom for NT page files and have never seen a disk this distressed. Whether that applies to Win2k Server I'll leave to someone else to answer.
