It's a ridiculous code but if your main.prg calls this form you could reproce the problem.

Not once, but 10 ou 50 times.
Put a command at GOTFOCUS to autoclose the form.

Make a EXE and starts call this program.

call MAIN.EXE
call MAIN.EXE
call MAIN.EXE
call MAIN.EXE
call MAIN.EXE Ops, a error...
call MAIN.EXE

If you call 50 times could be that 4 or more erros fire.
It's not a constant.

Sometimes more, sometime less...

After remove and change this old code at my init form event the error persistis. Something else also produce this problem.

But it's a example to reproduce the error.

I hope you find a fix to this bug.
