I have had this situation with really LONG patient surveys. We have split the data into several tables. I think that if I had it to do over again I would try the split out any child info possible though I don't think there really were any, I also would think about combining some fields that were stored as logical into a binary and use some binary functions. Sometimes you just need that many fields.
