Hey Phil. I think that there's no single standard. Personally, for local travel time, I do not charge. But, I have another colleague who also tends to bill hourly, and he does in fact charge for travel time during visits. So I don't think it's unheard of....
Overnight travel is something else entirely. The only time I did this with an hourly client, I charged for my "day" (assuming 8 hours), added another 20%, and also had them pay for plane tix and hotel. The 20% addition was intended to account for the fact that I imagined that I would discuss work over dinner with the client, would look up a few things back at the hotel, would review their documents on the place, etc., but didn't feel comfortable billing them for every minute of work.
